Cisplatin is one of the most commonly used drugs in combined chemotherapy. Its chemical name is cis dichlorodiamine platinum. It is an inorganic metal complex, which can dissociate cross bind with DNA of cancer cells, thus destroying the function of DNA. It may form cross-linking with DNA, DNA and protein, and inhibit cell mitosis. It is a cell cycle nonspecific drug.
Besides anticancer, it can also inhibit lymphocyte transformation and has immunosuppressive effect. It can be used as a metal complex of anticancer drugs.
Function
Cisplatin is a potent platinum-based anti-neoplastic agent that is believed to form inter- and intrastrand DNA adducts which activate signaling pathways culminating in apoptosis. The compound has also been shown to induce apoptosis through caspase-3 activation and XIAP expression. Also shown to cause immunosuppression which is thought to be followed by an increase in the host immune response. Platinated DNA adducts also enhance poisoning of DNA topoisomerase I by camptothecin (sc-200871). Cisplatin was the first member of a class of anticancer drugs, which now include carboplatin (sc-202093) and oxaliplatin (sc-202270), that form DNA adducts resulting in apoptosis; however, these molecules also bind other biological molecules such as enzymes which might play a role in their mechanism of action.
Application
The range of action of carboplatin is generally similar to that of cisplatin. The main advantage of this new analogue is its difference in toxicity.
At present, carboplatin is used as palliative treatment for recurrent ovarian cancer, including patients who have previously used cisplatin. Clinical studies have also shown that carboplatin can produce an objective response to head and neck tumors, small cell lung cancer and testicular seminoma.
At present, carboplatin is mainly used in the treatment of small cell lung cancer, ovarian cancer, testicular cancer, germ cell tumor, thyroid cancer, nasopharyngeal carcinoma, cervical cancer, non-small cell lung cancer, esophageal cancer, seminoma, bladder cancer, mesothelioma, pediatric brain tumor and other head and neck cancer. Cisplatin can be used in patients who cannot tolerate cisplatin due to renal impairment, refractory vomiting, hearing loss or neurotoxicity. It can be used alone or in combination with other chemical drugs. It can also be used in combination with surgery and radiotherapy to improve the curative effect.
